Conference Registration Form

By clicking on the link below you will open a registration form.  Please print and complete the form and mail to the address on the bottom.  If you wish, at a later point we will have the means for you use your credit card online to pay for the conference.

Instruction for completing the form:

In the top section please complete all areas. Make sure to mark if you are the elected state representative or not for your Advisory Council.  If you are your council's elected state representative KOT will reimburse your council up to $100 plus mileage for your attending the conference.

Conference Registration Options:  Please choose one or more of the options.  Please note that KOT actively and fully supports the Advisory Council sending at least two people to the conference at the expense of the council.  All cost are based on double occupancy.  We will find you a roommate unless you specify one on the form.  If you would like to have a single room, please mark the single room block.

We have made provision for three or four people to share a room.  If you would like to take advantage of that option you must find the roommates.  KOT will not.  Please include all the roommates on the form when you send it in.

We are working on activities for spouse who would like to come but are not involved in Kairos.  Those activities will be published as they are firmed up.
